At Coordinated Connections, we believe that keeping connected with other people that share your interests and goals are important and can boost confidence, life skills, creativity, mental health, cognitive skills, and physical motor skills.

We offer a range of in-home care, disability support, emotional and mental health support, and developmental support services to suit your needs. Our extensive range of services includes assistance with daily activities, household tasks, community access, assistance to participate and build community connections, and childhood play development to help you grow and maintain your independence.

Our support services prioritise the individual, their needs, wants, and preferences. All of our support services are individually tailored, but you can find an overview of each service we provide below.

Mental Health Support Workers

Developmental Support Workers
Developmental support workers support families with children with developmental delays to build skills to engage in daily activities within home and community environments through collaboration with families and allied health professionals. We understand that a child with complex needs can take up a lot of time, so our support workers can assist to lighten the load for parents and carers, allowing more time to complete other necessary tasks or just time to take a break.
